Kansas Customer Water System Service Line Mandatory Inventory Questionnaire

8/29/2024

Dear Water Customer,

The U.S. Environmental Protection Agency (EPA) recently issued regulations requiring all public water supply (PWS) systems to develop a Lead Service Line Inventory (LSLI) of both water system owned and your privately owned service lines. Your PWS system must submit this inventory to the Kansas Department of Health and Environment (KDHE) by October 16, 2024.
While these contaminants are not in the source water, they can enter tap water through the corrosion of service line piping and older household plumbing materials.

The City of Carbondale is sending this survey to all customers asking that you complete and return the form that will arrive via mail, or complete the survey online at https://s.surveyplanet.com/k3qgcalb. Please complete the paper or online survey and return it to the city as soon as possible, or at latest, by October 10, 2024.

We thank you for your cooperation in helping us with this to continue to serve clean and safe drinking water to our customers. Please take a few minutes to complete this questionnaire to the best of your ability and return to City Hall or complete online and your responses will be added to the inventory database.  Questions should be directed to City Hall at 785 836 7108.
